We ask that you complete your registration forms completely and thoughtfully as we want to make your first visit with us as thorough as possible. When completing an Account Information form for a child, please be sure that the account holder is an adult. We include a Sleep Questionnaire for adults because we have found that many of our patients are not getting restful and restorative sleep and there are ways that dentistry can help.
Your initial appointment will be with a hygienist for a cleaning, during which comprehensive oral, dental, and periodontal (gum) exams and needed x-rays will be completed. We utilize a diagnostic laser called a DIAGNOdent to help locate the early stages of decay in the bottoms of grooves and pits. This allows us to treat the decay early before much tooth structure is lost. A smaller filling is needed and the rest of the tooth is preserved! We offer the non-invasive VELscope to screen the soft-tissues of the mouth for any abnormalities–some of which could be precursors to cancer. Our doctors review Xrays and Intra-oral photos after patient hours so they don’t have to rush through the process. Then at your second visit, the doctor will discuss your dental needs and the treatment options to address them.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the first visit.
Please alert the office if you have a medical condition that may impact your dental appointment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics for pre-med.)
If your previous dentist has taken recent x-rays (within the last 3 years for a “full-mouth series”), you may request that they mail a copy of them to our office. We can accept e-mailed records at bridgewaterfamilydental@gmail.com. If additional films are necessary, they can be taken by us.
